An in-home health visit is an assessment to evaluate your overall well-being. Your health plan is providing this service to you to gather a total and precise picture of your current health. As a part of your preventative care, the in-home health visit gives you the chance to spend an hour of one-on-one time with a doctor, nurse practitioner or physician’s assistant to discuss any questions or concerns you may have regarding your health.

The medical professional will go over your personal and family health history and will measure your height weight and blood pressure. They will also screen for chronic conditions including but not limited to Diabetes, Chronic Obstructive Pulmonary Disease (COPD), and heart disease. The doctor or nurse practitioner is also there to help you understand every aspect of your health including education, counseling, and referrals for important covered screenings and preventative care.

A bone density scan is a non-invasive scan of your foot that helps determine if you are at risk for Osteoporosis. The test takes about 5 minutes and measures the density of your bones. Your health plan wants you to have this test done especially if you have suffered from a recent fall or fracture.
Your in-home health visit provides you, your doctor, and your health plan with a snapshot of your current health. It is a baseline for future yearly visits and allows your primary care physician to have the most up to date and accurate picture of your health. The in-home health visit with a MedXM doctor, nurse practitioner or physician assistant can take anywhere from 45 minutes to an hour, depending on how many questions you have and your current health condition.

If the MedXM doctor, nurse practitioner or physician assistant finds anything worthy of alarm, they will do everything necessary to take care of your condition whether it be calling 911 or passing along urgent information to your primary care physician. Otherwise, the information gathered is used to help prevent any future illness or emergency room visits.
MedXM only sends certified and credentialed doctors providers to your home. If you are uncomfortable meeting in your home, the MedXM provider can accommodate you by meeting on your patio or porch. You are also able to have a friend or family member with you at the time of the visit.
